Nie ma bardziej popierdolonego kraju w kwestii papierologii niz USA. Wklejam wam ponizej "skrocona" instrukcje od agenta tego co trzeba przygotowac na przyjazd. Jedziemy tam tylko na 2-3 doby, zeby zatankowac. Wypelniania papierow jest na kilka ladnych godzin.

A. IMPORTANT INFORMATION

* Current eta LGB/LAX pilot station
* Fwd, Aft & Air drafts on arrival (mast up and mast down), and estimated departure drafts if known
* Confirm all navigational equipment in good order
* Any crew to doctor, pls ask all crew a few days and then 24 hrs prior to arrival, this is very important

* Confirm all services needed this port call
* Confirm vessel has valid Federal COFR, Calif. COFR, and Calif. Oil-Spill Contingency Plan
* Does vessel require any publications/charts on arrival
* Does vessel have any outstanding USCG deficiencies
* List of all crew onboard without US Visa

B. REQUIRED INFORMATION FOR CUSTOMS ENTRANCE (ONLY IF VSL TO BE IN PORT +24 HRS)

* Copy of vessel particulars, crew list, ports of call list (one year)
* Copy of vessel registry
* Load Line & Safety Equip. certificate expiration date
* Total crew & pax on arrival & departure
* Name of next TWO ports after departing LA/LB
Copy of latest AGM free certificate on board, even if old from last season. USDA will want to see if vsl in compliance in the past.  (If your vsl called far east ports)*

C. ELECTRONIC NOTIFICATIONS (eNOA & eNOD)
ARRIVAL * submit to enoad@nvmc.uscg.gov
1. FOREIGN to USA voyage, eNOA submitted six days (or at minimum 96 hours) before arrival pilot station.
2. USA to USA voyage +96hrs, eNOA submitted six days (or at minimum 96 hours) before arrival pilot station.
3. USA to USA voyage -96hrs, eNOA submitted before departing your last USA port (AND minimum twenty-four hrs before arrival pilot station).
4. Note that ur ETA should be reported to pilot station, not berth.
5. Norton Lilly should be listed as the PIC in the eNOA section labeled “24 hr Point of Contact”
DEPARTURE * submit to enoad@nvmc.uscg.gov
1. USA to FOREIGN voyage, submit initial eNOD at least one hour before departure from berth/anchorage.
2. USA to USA voyage don’t not require eNOD.
IMPORTANT: 
1. Vessel must copy xxxxx with EVERY email sent to enoad@nvmc.uscg.gov
2. Vessel must update an eNOA/D only if ETA/D has changed more than six hours from the last submission.
D. CUSTOMS and IMMIGRATIONS INSPECTIONS (ONLY IF VSL TO BE IN PORT +24 HRS)
All foreign arriving vessels will have US Customs inspections. Only the vessel’s agent, U.S. Customs and U.S Coast Guard authorities may board your vessel upon arrival from a foreign port. Crewmembers may not disembark prior to immigration inspection unless permission is granted by CBP and no one may board your vessel until clearance is granted by CBP and /or the vessel’s agent. Any penalties resulting for failure to comply with these regulations will be for vessel owners' account.
All foreign arriving vessels will have US Customs inspections. Pls have all the below ready for CBP inspection immediately on arrival at berth.
1. The following original documents

* Vessel registry
* International Load line certificate
* International Tonnage certificate
* Certificate of Financial responsibility (Federal version)(COFR)
* Latest AGM free certificate, even if from last season
* Safety Equipment certificate with supplement
* US tax receipts
* Last port clearance
* Garbage log/records
* If owner on registry is different from operator on the Federal COFR, we will need a bridge letter that connect the two parties

2. Three copies the following forms (each document originally signed & stamped).

* Crew List CBP form# I-418 (in alphabetical order only),
* Crew Effects List CBP form# 1304,
* Ships Stores List CBP form# 1303
Narcotics* must be on stores form or vessel is subject to 5,000 USD fine
* Port of Call list
* Shore pass CBP form# I-95 for all crew onboard
* Shore pass CBP form# I-94 for all pax onboad

3. Pls make ready one copy of passport for any crew without an US VISA for presentation to CBP before boarding.
E. BALLAST WATER REPORT (GLOBE FORMS WITH FDZ. FORMAT WILL BE REJECTED)
All vessels are required to submit a ballast water report not less then 24 hours before arrival to EACH USA port. Vessels that intend to discharge ballast in USA ports/anchors are required to exchange ballast water at sea not less then 200 miles from land and be able to provide clear documentation if requested while in the USA port. Submit report to ALL the following parties:
1. nbic@ballastreport.org or fax to 301-261-4319
2. bwform@slc.ca.gov or fax to 562-499-6444

F. CALIFORNIA HULL SURVEY (GLOBE FORMS WITH FDZ. FORMAT WILL BE REJECTED)
Vessel to submit copy of the “California Marine Invasive Species Program Hull Husbandry Survey” once per calendar year while visiting any State of California port (or within 45 days of the first port call). Submit report to ALL the following parties:
1. bwform@slc.ca.gov or fax to 562-499-6444
2. xxxx or fax to 562-628-1025

G. AGRICULTURE INSPECTIONS (ONLY IF VSL TO BE IN PORT +24 HRS)
All foreign arriving vessels will have an Agriculture inspection. CBP officers will inspect the upper deck for any loose garbage immediately upon the vsl’s arrival at berth. Pls ensure all galleys, dinning, and storage areas are properly maintained and well keep. All garbage receptors should have proper lining and be covered at all times. Inspectors may carryout random inspections of crew common/personal quarters. Vessels are reminded NO fruits/vegetables are allowed in crew member's cabin at any time while vsl is in port.
H. GREEN FLAG PROGRAM (LONG BEACH ONLY) 
In order to be in compliance with Green Flag incentive program, vessels entering or leaving the Port must observe a 12-knot (max) speed limit in a zone that extends 40 and 20 nautical miles seaward from Point Fermin.
The speed of every vessel in the speed reduction zone is measured and recorded by the Marine Exchange of Southern California. We greatly appreciate your attention and cooperation with this important air quality improvement program. Please contact your vessel planner or port captain to confirm which program (40 or 20) to comply with.

J. FUEL REGULATIONS (CARB AND ECA)
The State of California has very strict fuel regulations (CARB) which start 24 nautical miles from the shoreline and covers all ports in the State of California(this includes 24 NM off the Channel Islands). The minimum fine for non-compliance is $45,000. Please urgently contact with your vessel owners and/or the local agents to obtain more details on how to comply. Again the vessel must have onboard BEFORE ENTERING CALIFORNIA WATERS the correct fuels. The North American ECA is also in place off the coast of all Canada and the U.S.A. Please note you will need to operate on LSFO in the ECA zone until you reach the CARB that requires switch over to LSMGO or LSMDO. If you need additional information please ask.
1)       200 NM off the coast of the United States and Canada vessels must switch from HSFO to LSFO (.1% or less) per the ECA.
2)      24 NM off the State of California vessels must switch from LSFO to LSDO or LSGO (0.1% sulfur or less) (Phase 2).
K. SMOKE EMMISIONS
The State of California also has very strict smoke emissions regulations. Vessels are allowed to emit black smoke for maximum three minutes per hour while in port. There are many local agencies that patrol the port/harbor, so please take necessary precautions to avoid emitting black smoke. If the emission is not avoidable, please contact the following local authorities to notify them in advance and request relief. The request will not always be approved, however, the authorities greatly appreciate the vessel to show the effort of compliance.

* Contact VTS on channel 14 for life boat permission and notify pilots

M. SHIPS MAIL AND PACKAGES TO BE LANDED

* All landed ships mail must have a landing form, one for Master and one for Agent or mail cannot be taken off vessel, this is for accounting purposes

N. MAIN ENGINE IMMOBILIZATION

* C/E may conduct main engine immobilization while alongside after you send email to lalbcommandcenter@uscg.mil with reason for the immobilization, terminal name and expected duration
* Do not expect email back from USCG unless they have questions, i.e. your first email was not detailed enough, permission is granted
* We suggest you email this at least 24 hrs prior to arrival

O. OWNERS ITEMS (IMPORANT)

* For owners items please send owners full style for contact and billing so we do not have to email your good vessel for same information post sailing
* All requests verbally asked to agent on arrival or via telecom must be followed by an email from vessel to confirm

P. PAINTING

* Allowed with no formal permission as well. Pls be warned California has strict environmental regulations, spillage in water is very serious, paint CAREFULLY at your own risk. You must have a copy of the Port of Los Angeles or Long Beach best management practices on board. Please email a request for this if you do not already have a copy on board

Q. BUNKERING ONLY OPERATIONS – GUIDELINES/INFORMATIONS
Basis your port call is scheduled for less than 24hrs in port, the local Customs Officers will not board your vessel for inspections. However, all persons that need to visits your vessel must be approved by Customs before boarding. Please provide the following details to agents 48hrs before arriving LGB in order to make submission for Customs approval.

On departure vessel is requested to send following times/conditions to agents in order to complete the final statement of facts:

Moglbym zostac i pojechac, ale nie widzi mi sie robic 9 tygodni, wiec odklepie swoje 5 i jade do domu. Polece do korei nastepnym razem.
